Filing a Claim with The Standard

File your Paid Leave Oregon claim with The Standard, not with the state of Oregon.

1. Notify your supervisor of your need for time off as soon as possible. 

For foreseeable events, like a birth or a planned surgery, give your supervisor at least thirty days' notice. 

In an emergency, notify your supervisor that you plan to use Paid Leave Oregon benefits within twenty-four hours of starting your leave. Follow up with a written notice to your supervisor within three days of starting leave. 

If you do not give the required notice, your first weekly benefit may be reduced. 

2. Call The Standard at 1-800-242-1888 Monday through Friday between 5 a.m. and 5 p.m. Pacific to initiate your leave claim.

Information you’ll need to provide: 

  • Your name and Social Security number 
  • Employer name (University of Oregon) 
  • UO’s policy number: 764913-A 
  • Reason for leave 
  • Name and contact information for your medical provider 
  • Last date you worked or plan to work 
  • Expected duration of your leave 
  • How you want to receive payments (paper check, Zelle, or direct deposit). 

3. Send necessary information and documentation to The Standard.

Within 24 hours of your intake call, The Standard will send you a letter via email and U.S. mail outlining your request and any documentation needed to support your claim. Please return the requested information by the due date indicated in your letter

4. The Standard will review your claim and email you their determination.

Most claims receive a decision within five business days after The Standard receives all required information and documentation. If anything is missing or incomplete, The Standard will notify you.

5. Submit the PLO Employee Response Form to the leaves team in Human Resources.

Look for an email from noreply@ukg.uoregon.edu with a link to complete and submit your form. Remember to check the “Other” tab in your inbox if you have Focused Inbox turned on. 

On this form, you’ll elect whether to receive “top off” payments from your vacation and/or sick leave balances, to maintain your current pay up to a maximum allowed amount. See the next step. 

Complete and submit the form within five days to avoid your claim being delayed or denied.

6. If you elected to top off your PLO benefits using your accrued leave, watch for an email from hrleaves@uoregon.edu

We will send you this email after you receive your first payment from The Standard.

The email will confirm:

  • Whether you are eligible for top-off payments.
  • The amount of your top-off payments.
  • The number of leave hours you will need to report each month in UKG, to receive your top-off payments.

7. Monitor the email you provided for leave-related messages, so you can complete and return all required information.

Keep an eye out for messages from 

  • The Standard. 
  • HR Leaves. 
  • Your supervisor or unit payroll administrator. 

If your email inbox has a “Focused” and an “Other” tab, make sure you check the “Other” tab. Sometimes, messages from new senders outside the UO are delivered there. 

8. If you chose an electronic payment method (direct deposit or Zelle) The Standard will prompt you with a text message to accept each payment.

If you don't accept the payment within three days, you'll receive a paper check.

You will receive a text message prompting you to log in to PNC Bank's system to accept each payment.

9. If you are using top-off leave, report your top-off leave in UKG monthly.

Report the number of hours instructed in the email from Step 6 above.

Follow your unit's usual monthly payroll process.

If your unit does not use UKG for monthly timekeeping, your unit's payroll administrator will enter your top-off hours in UKG on your behalf. You will need to let them know which type of accrued leave you'd like to use (vacation, sick, personal, comp time, etc.).

10. If you are on intermittent leave, report your intermittent leave to The Standard every week by calling 1-800-242-1888.

You can report intermittent leave to The Standard via automated phone or text, 24/7.

Intermittent PLO leave must be taken in full day or full week increments. 

You do not need to report if you take zero days of leave in a week.

11. If your situation changes, notify The Standard.

Benefits Calculator

The Standard provides a calculator to help employees estimate their weekly PLO benefits. Actual benefit amounts may vary once you have submitted your claim and it has been adjudicated by an examiner. The tool is not a guarantee of eligibility nor benefit amount.

In the calculator, select Oregon as your work state, enter your estimated leave start date, and then your hourly wage or annual salary, then click Calculate.
